Search on www.amazon.co.uk for the book you are reviewing USING THE ISBN13 (for example 978-1234567890) on your book. The 'product details' give the information which you will need to complete the submission. Please note that the ISBN-10 is rarely the ISBN-13 with the leading '978' lopped off!
The simplest way to complete the form is to use 'find and replace' to insert the information (in upper-case font) into all the places where it is required at once. All text in upper case must be removed before submission. If you don't use find and replace – all text in the same colour requires EXACTLY the same information – don't worry about what it says in the code at the side.
