Changes

The simplest way to complete the form is to use 'find and replace' to insert the information (in upper-case font) into all the places where it is required at once. All text in upper case must be removed before submission. If you don't use find and replace – all text in of the same colour text requires EXACTLY the same information – don't worry about what it says in the code at the side.

DON'T DELETE ANYTHING IN LOWER CASE (other than 'see note...') AND TYPE THE RELEVANT INFORMATION BESIDES THE '=' WITHOUT DELETING ANYTHING WHICH COMES BEFORE OR AFTER IT.

You will need to copy everything from HEADING to SUBMISSION ENDS onto into a Word document (or however you handle word processing) or and then into the body of an email and then which should be [mailto:suemagee@gmail.com email this sent to me]. Please put the heading as below in the subject line.
